My role is to support NDIS participants in working towards their NDIS goals. Services include
- Connecting participants to NDIS providers as well as services in the community
- Working closely with all stakeholders to ensure transparency, accountability, communication and collaboration.
- Ensuring consistent service delivery for participants in all areas of their supports.
- Provide assistance to resolve issues and unexpected situations.
- Developing of a comprehensive support plan
- Explore and organise new services that will meet your changing needs.
- Monitor spending throughout the plan period to ensure continuity of supports.
- Coordinate the end of plan reporting process for the annual review
